Please prepare the following documents before filling out the online admission form.

NEW AND RETURNEES: ( for Students from Philippine Schools )

Form 138 / Report Card
Certificate of Good Moral Character
Birth Certificate ( PSA - authenticated for Filipino Citizens )
ESC Certification ( for Grades 8 - 12, if available )
ESC Application & Contract ( for Grade 7 students )
Qualified Voucher Recipient (QVR) Certificate for Grades 11 & 12 students ( if student applied for SHS Voucher )
Student ID Picture (in proper attire, white background)

CONTINUING STUDENTS:

Complete Exit Clearance for SY 2022-20223

NEW AND RETURNEES: (Filipino Students from Foreign Schools)

Original and certified photocopy of School Records
Certificate of Good Moral Character from the School Last Attended
Certificate of Graduation (for those who finished the elementary level)
Copy of Birth Certificate or its equivalent (if born in the Philippines, Birth Certificate must be PSA-authenticated)
Data Page of the Passport
For those with dual citizenship, Two valid passport biopages (foreign and Philippine passports)
For those with dual citizenship, Certificate of Citizenship Retention/Re-acquisition (from PFSP) or Certificate of Citizenship Recognition (from the BI)
Very Important:

a. School records, certificiates or any documents originally written in other languages should have a notarized English translation.

b. Scholastic records must be duly authenticated by the Philippine Foreign Service Post located in the foreign country where the student last attended. School documents must have apostille stamp or seal.

NEW AND RETURNEES: (from Foreign Students)

Certificate of Good Moral Character from the School Last Attended
Certificate of Graduation (for those who finished the elementary level)
Copy of Birth Certificate or its equivalent
Data Page of the Passport
Notarized Letter of Guardianship appointing Mountain View College or its school representative to be the Filipino Guardian of the child during the entire period of his/her stay at Mountain View College
Pay an application fee of $200
Pay Financial Guarantee Deposit of One Thousand Dollars ($1,000) which could only be withdrawn upon graduation or leaving the college permanently.
Very Important:

Note to all students from Foreign Schools:

All parents/sponsors have to send all the softcopies of the admission documents first to the Liaison Officer for assessment and application of the Certificate of Eligibility from the Department of Education Region X Office.
